چکیده

C hronic total coronary occlusions (CTOs) represent one of the final frontiers in the evolution of revascularization with percutaneous coronary intervention (PCI). Although CTO revascularization relieves symptoms, reduces ischemic burden, improves left ventricular systolic function, and attenuates remodeling, convincing proof of improved survival remains elusive (1–3). Ongoing randomized trials such as EXPLORE (Evaluating XIENCE V and Left Ventricular Function in PCI on Occlusions After STEMI; NTR1108) (4), DECISION-CTO (Drug-Eluting Stent Implantation Versus Optimal Medical Treatment in Patients With Chronic Total Occlusion; NCT01078051), and EuroCTO (European Study on the Utilization of Revascularization Versus Optimal Medical Therapy for the Treatment of Chronic Total Coronary Occlusions; NCT01760083) will augment our understanding of long-term CTO outcomes (5), but none of these trials is statistically designed to detect mortality differences. Estimates of the mortality risks associated with CTO and the potential survival benefits of CTO PCI therefore remain founded in estimates from robust observational series (6–11).

Current percutaneous recanalization of coronary chronic total occlusion.

Although successful recanalization rates of coronary chronic total occlusions have remained suboptimal in percutaneous coronary interventions, evolving techniques, including the retrograde approach, have raised hopes for better outcomes. With the advent of antiplatelet therapy and drug-eluting stents, along with conventional antegrade approaches, further progress can be expected in the "last fr...

Retrograde Approach for Chronic Total Occlusions: the Importance of Experience and Proctorship

Recanalisation of chronic total occlusions has remained suboptimal in the field of percutaneous coronary intervention (PCI). Hopes for a better outcome have been raised since its introduction and evolvement of new techniques, including retrograde approach. Along with conventional antegrade approaches this technique led to a success rate of more than 90% in experienced and chronic total occlusio...

The Hybrid Approach to Intervention of Chronic Total Occlusions

The "hybrid" approach to chronic total occlusion (CTO) percutaneous coronary intervention (PCI) was developed to provide guidance on optimal crossing strategy selection. Dual angiography remains the cornerstone of clinical decision making in CTO PCI. Four angiographic parameters are assessed: (a) morphology of the proximal cap (clear-cut or ambiguous); (b) occlusion length; (c) distal vessel si...

Percutaneous Coronary Intervention for Chronic Total Occlusions

I mpasses are common in everyday life. When they occur in coronary arteries as chronic total occlusions (CTOs), they may be difficult to treat. Successful treatment of CTOs sometimes requires a nonconformist approach based on “negative thinking,” which is described in philosophical circles as the mindset that “negates” the possibility of failure and replaces it with the potentiality of success ...

Acute myocardial infarction in an 80 year-old woman caused by left main occlusion with concomitant chronic total occlusions of right and left coronary artery: successful treatment with percutaneous revascularization.

Although acute left main coronary artery occlusion is rare, it carries a very high mortality rate. Following the encouraging results of trials testing the effect of primary percutaneous coronary intervention, more cases of left main stenosis are treated as culprit lesion in acute myocardial infarction. Few cases of primary percutaneous intervention on left main occlusion have been published. We...
